We will discuss the many barriers facing students transitioning from the juvenile justice system back to their communities, as well as how to build collaborative partnerships with key agencies, CBO and school partners that can assist with ensuring students are able to successfully transition back to their communities. The presentation will explain in detail why and how the Transition/Aftercare Model for Los Angeles County was created, the roles that school districts and school site administrators play in the transition process, and what school administrators can do to assist transitioning students reach their academic goals.
